The movie ends with the implication that Lefty was killed after being "sent for". In real life, the FBI intercepted Lefty on the way to being killed and arrested him. Sonny Black, however, was "sent for" and subsequently murdered, his body turning up a year later on Staten Island. The individual who had orchestrated his murder, Joe Massino, wasn't convicted until 2005. Lefty was convicted of conspiracy to commit murder, extortion, distribution of a controlled dangerous substance, and running an illegal gambling operation; he was sentenced to 20 years in prison, but received early parole in 1992 after it was discovered he was suffering from terminal cancer. He died of lung cancer in 1994.

L'action se déroule en temps réel à Paris. Cléo, une jeune et belle chanteuse plutôt frivole, craint d'être atteinte d'un cancer. Il est 17 heures et elle doit récupérer les résultats de ses examens médicaux dans 2 heures. Pour tromper sa peur, elle cherche un soutien dans son entourage. Elle va se heurter à l'incrédulité voire à l'indifférence et mesurer la vacuité de son existence. Elle va finalement trouver le réconfort auprès d'un inconnu à l'issue de son errance angoissée dans Paris.

Comment continuer à vivre lorsque la maladie vous guette ? Lorsque Cléo, une très belle chanteuse, apprend par une cartomancienne qu'elle est malade, la jeune femme tente d'abord d'oublier cette mauvaise nouvelle, en compagnie de son amie Angèle et de ses musiciens. Mais décidément, le coeur n'y est pas. L'indifférence, l'égoïsme et la méchanceté de son entourage lui font soudain prendre conscience de sa solitude. Très éprouvée, hantée par la peur de la mort, Cléo erre sans but dans les rues de Paris. Au fil de sa dérive, elle regarde un court métrage muet

Production/Genèse
Histoire de saison : Agnès Varda voulait filmer le 21 mars pour « Capter dans Paris le passage merveilleux de l'hiver au printemps avec les jardins passant du dessin à la plume à la peinture impressionniste ». Pour des raisons financières, le tournage ne put commencer que le 21 juin. Le film bénéficie quand même de très belles images d'un blanc cotonneux lors des scènes tournées à 6 heures du matin au Parc Montsouris pour une action censée se passer à 18 heures…

Les Fiancés du pont Macdonald : pour ce mini-film dans le film, burlesque, mais néanmoins clé de voûte de l'histoire de Cléo, Agnès Varda a fait appel à des copains. Les Fiancés, c'est le couple Godard-Karina, vedette de la Nouvelle Vague et entouré de ses comparses. On aperçoit Frey et Brialy, le cinéma vétéran étant représenté par le couple Delorme-Robert. Pour Varda, cela reste « Un souvenir qui symbolise la Nouvelle Vague telle que nous l'avons vécue, l'imagination au pouvoir et l'amitié en action. » (sic).
